WORKSHOP PROPOSAL REQUIREMENTS

1) Please complete the online workshop proposal form by pressing the "submit" button at the end of the page after completing the online questionaire.

2) Send by fax or email:
• Examples of handouts that will be used (may be mailed).
• Any other relevant materials or information (website, etc.).
• Listing of other workshop presentations (when, where, and to whom).
• Curriculum vitae (or list of relevant agencies and populations you've worked with).
• Signed presenter agreement form (one per presenter).

In an effort to empower and promote the inclusion of direct service and grassroots experts along with professional trainers, various presentation formats are being offered. Applicants are encouraged to include more than one possible presentation format. Can you present your topic in either a workshop or a panel format?

If so, please check those presentation formats that might be applicable for your topic (with a '1' or '2' for ordered preference):

Out of respect for advocates who wish to receive training in Spanish - bilingual sessions will not be accepted.
